COURSE EVALUATION
Submit this form within 24 hours of course completions. NO EXCEPTIONS. You will be sent an email indicating that you did not meet the time deadline, as well as if you didn’t meet the hours required based upon the zoom report. All nursing information is transcribed to the Alabama Board of Nursing by HRO. All other participants’ rosters are sent to the Alabama Boards for each discipline quarterly. One certificate provided for each class that you have attended. Duplicate certificates are $10.00 for each class.
